Supported Living is a service under the DDA Community Pathways Waivers that provides individuals with a range of individualized community living supports to help them live independently in their own homes. These services are designed to promote self-determination, choice, and independence while respecting each person’s unique goals, strengths, and preferences.
Supports may include assistance with daily living skills, household management, community integration, and personal development based on the individual’s needs. Services are delivered in a flexible, person-centered manner and can be adjusted as needs change over time. Our goal is to empower individuals to maintain independence, build confidence, and actively participate in their community while receiving the right level of support to succeed.
